We are seeking a permanent full time Charge Nurse for the OPAL (Older Persons Assessment and Liaison) ward at Palmerston North hospital.  The position is Monday-Friday, excluding statutory holidays.

The OPAL Ward is a 31 bed acute care ward which provides specialist geriatric and multidisciplinary team led care for older people with frailty.

OPAL ward goals are to improve functional outcomes, reduce in-hospital complications and facilitate timely transfer of care back to the community for older adults with frailty, by promoting early mobilisation to prevent deconditioning, focusing on what's important to the patient and developing an integrated and coordinated care plan in conjunction with the patient, whānau and primary care team.

The OPAL unit model of care is based on comprehensive geriatric assessment (CGA), which is a multidimensional, interdisciplinary, diagnostic process focused on determining an older person's medical, psychological and functional capability in order to develop a coordinated and integrated plan for treatment and long term follow up

The Charge Nurse leads the team in ensuring high quality patient focused nursing care is delivered. The Charge Nurse leads the development, implementation, monitoring, and evaluation of patient focused nursing care, promoting excellence, continuity and collaboration within the inter-disciplinary team and with families, to improve patient health outcomes.  The Charge Nurse has the opportunity to contribute both at an operational and a strategic level to developments within the service.
