As part of
a diabetes CNS team, you will be working collaboratively with other
multi-condition hub members and other health professionals to provide
coordinated, tailored health promotion to patients, family, whanau and
caregivers. You will also provide
specialized education and mentoring of other health service professionals.
You will be
involved with initiating and or participating in evidence-based service
improvements, quality enhancement, policy development and building of staff
capability.
Clinical aspects
of this role will include face to face clinics with patients with diabetes
across the age continuum.
