Great opportunity to work for our not-for-profit client, whose main focus is to provide people with disabilities access to an array of services designed to make life easier and more meaningful. This organization has been around since 2002 and works with more than 13,000 disabled people and their families across the wider Auckland region from Mercer to Wellsford.

Working as a Pathways Navigator you will be responsible for carrying out Pathways activities, to ensure people with needs, and their families, receive a professional and empathetic approach for prompt and effective access to services that meet their needs/goals.

Responsibilities include:

  • Make initial contact with referred clients/families applying for the Ministry of Health eligibility criteria to all referrals
  • Seek information proactively to establish eligibility and determine urgency and risk
  • Advise eligibility for different services and refer accordingly
  • Provide information, educate and advise contacts and clients about the eligibility, supports, services and access a wide range of providers and referrers
  • Identify and prioritize client support needs whilst educating client on user driven process
  • Make assessment bookings with clients and allocate initial assessment to the relevant staff member

To be considered, you will have:

  • 2 years- experience in a Clinical or Disability support position
  • Health or disability related qualification
  • Ability to communicate effectively in a timely manner that reflects sensitivity
  • Excellent phone and administration skills
  • Clinical experience preferred
